Self-Pay / Direct Care Patients
In-person & tele-visit appointments available for both new and existing patients.

New Patients
- Previous patients establishing a care, or new patients requiring one service treatment (e.g., hormone therapy & weight loss, mental health consults):
- Tele-Visit: $200
- In-Person: $250
New primary care, Medicare, and pre-op patients must complete their initial visit in-person: $250–$400 (based on time and complexity).
Existing Patients
- Simple Acute Care / Routine Follow-Up
- Includes medication refills, basic concerns, or follow-ups not requiring lab review or extensive discussion
- Tele-Visit: $110
- In-Person: $150
- Complex Visits
- Includes management of two or more active conditions, lab orders/review, or new medication starts
- Tele-Visit: $150
- In-Person: $200
- Controlled Substance Management
Patients prescribed controlled medications must be seen in person at least twice per year. Additional in-person visits may be required depending on individual treatment plans.

Medicare Patients
Lumina Medical accepts Medicare and provides services for Medicare beneficiaries.
If your specific Medicare plan is not accepted by our practice, or if you choose to receive services not covered by Medicare, you may elect to pay out-of-pocket.
In such cases, you acknowledge that no claims will be submitted to Medicare for those services.
Note: You may be asked to sign a waiver acknowledging your choice to receive and pay for non-covered services voluntarily.
Financial responsibility
- Payment Due:
- "Payment for services is due at the time of service unless other arrangements have been made in advance."
- Insurance Responsibility:
- "Patients are responsible for co-pays, deductibles, and co-insurance amounts as specified by their insurance plan."
- Out-of-Network:
- "If we are not in-network with your insurance, payment in full is expected at the time of service."
- Non-Payment:
- "Failure to pay within [number] days of the billing date may result in the account being sent to collections."
- Insurance Changes:
- "Please notify our office of any changes to your insurance information before your next visit."
