At Lumina Medical, we are committed to providing high-quality care and clear communication about financial responsibilities. As a new practice, we are actively working to accept as many insurance plans as possible, including most major commercial insurances and Medicare. Please contact us at (214) 267-9109 for

Patients With Insurance

If you are using health insurance for your visit:

  • I understand that it is my responsibility to provide accurate and up-to-date insurance information for each visit.
  • I authorize Lumina Medical to bill my insurance provider for services rendered.
  • I am responsible for any copayments, coinsurance, deductibles, or non-covered services as determined by my insurance plan.
  • I agree to pay any balance not covered by my insurance and understand that failure to do so may result in my account being sent to collections.
  • I understand that certain administrative services (including but not limited to school/work forms, FMLA, disability forms, or letters) are not covered by insurance and will incur a $30 processing fee per form.
  • I understand that I may receive separate bills for services such as labs or imaging, which may be processed by outside providers.

Cash-Pay / Direct Care Patients

Visit Types & Cash Rates

For patients paying out-of-pocket (without insurance). Prices reflect the complexity and type of visit.

 If you are paying out-of-pocket:

  • I understand that Lumina Medical offers transparent, flat-rate fees for visits and services under the Direct Care model.
  • Payment is due in full at the time of service unless otherwise arranged in advance.
  • I acknowledge that I will not be submitting claims to insurance and Lumina Medical will not bill my insurance on my behalf.
  • I am aware that labs, imaging, or referrals may involve additional costs if provided by third parties, and I am responsible for those payments separately.
  • As part of our community care model, Lumina Medical may offer a no-cost initial consultation to discuss services and determine if our practice is a good fit. This consultation does not include diagnosis, treatment, or medical management.

Existing Patients

  • Simple Acute Care / Routine Follow-Up
  • Includes medication refills, basic concerns, or follow-ups not requiring lab review or extensive discussion
  • Virtual: $75
  • In-Person: $150

  • Complex Visits
  • Includes management of two or more active conditions, lab orders/review, or new medication starts
  • Virtual: $150
  • In-Person: $200

  • Controlled Substance Management
    Patients prescribed controlled medications must be seen         in person at least twice per year. Additional in-person visits may be required depending on individual treatment plans.

New Patients

Returning Patients Previously Seen by Sahba Yazdani, and New Patients inquiring on treatment (Hormone Therapy & Weight Loss, mental health consults)

  • Virtual: $ 200
  • In-Person: $250

New Patients with Complex Medical History and Pre-operative new visits
 Need to schedule in-person visits for a thorough evaluation.

  • $250–$305 (based on time and complexity)

Functional Medicine & Health Coaching

Most insurance plans do not cover functional medicine visits or services provided by a health coach. While we will make every effort to assist you with documentation and billing codes to potentially offset some of the costs, it is the patient’s responsibility to verify coverage and benefits directly with their insurance provider.


Health Coach Services & Rates

  • Initial Functional Medicine Consultation: $100
    Focused on overall wellness goals, nutrition, lifestyle, and health history.
  • Condition-Specific Consultation: $150
    For patients seeking guidance on a specific issue (e.g., A1c, cholesterol, digestive health).
  • Follow-Up Visits: $100 per session
    Ongoing support and progress check-ins.
  • Comprehensive Functional Medicine Packages
    Includes a personalized plan, relevant labs, supplement recommendations, and ongoing visits over 3–6 months.

Medicare Patients

Lumina Medical accepts Medicare and provides services for Medicare beneficiaries.

If your specific Medicare plan is not accepted by our practice, or if you choose to receive services not covered by Medicare, you may elect to pay out-of-pocket.

In such cases, you acknowledge that no claims will be submitted to Medicare for those services.


**You may be asked to sign a waiver acknowledging your choice to receive and pay for non-covered services voluntarily.
